Meet Yasmin Polivick, M.D., Board-Certified in Internal Medicine
Dr. Yasmin Polivick earned her medical degree from the University of Illinois College of Medicine in Chicago, where she received the Bamberger Scholarship, and completed her internal medicine residency at the University of Illinois Hospital. For the past seven years, she has practiced primary care at Endeavor Health, caring for patients across the full scope of internal medicine, from annual exams to the management of complex, long-term conditions. She will continue to be on staff with privileges at Endeavor Health.
She founded a concierge medicine practice because the conventional model no longer left room for the kind of medicine she trained to practice. Short appointments, long waits for appointments, overloaded schedules, and patient panels of over 2,000 patients make it difficult for any physician to know a patient well enough to help them fully.
Dr. Polivick will now keep her practice deliberately small, a fraction of the number of patients in standard primary care, which gives each member her full attention and dedicated time for a thorough conversation. She resonates with the concierge model because it allows her to care for the whole person, not just one symptom or number on a lab result during a rushed appointment.
Her approach is rooted in prevention and a strong conviction that the best medicine comes from a physician who knows you personally and stays with you over the years. How does the membership work?
Membership is an agreement that gives you direct, ongoing access to Dr. Polivick. You pay a yearly or monthly fee to become a member, and in return you get same-day or next-day appointments, unhurried visits, and the ability to reach your doctor directly by phone, text, or message whenever a concern arises. Your existing health insurance continues to cover your medical care as usual.
How much does the membership cost?
Individual membership is $3,000 per year. Couples and members’ adult children or parents may join at a reduced rate, and founding members who enroll before opening receive priority as the practice fills. Membership will be capped. The fee covers your access to Dr. Polivick and the personalized attention that comes with membership. It is separate from your health insurance, which continues to cover your medical care as it always has.
How do insurance and Medicare work?
You will still use your insurance or Medicare in the same way you would at a traditional primary care practice.
Foundation accepts most major insurance plans, including Medicare. Dr. Polivick will bill your insurance for services that are covered by your plan (e.g., annual exam). You will not be charged more than your insurance pays. Dr. Polivick will order lab work, imaging, and refer to specialists at facilities that accept your insurance whenever possible, and they will bill your insurance as usual.
Your annual membership is separate from insurance/Medicare and covers the access, time, convenience, coordination, and personalized care that insurance/Medicare does not pay for.
If Foundation is out of network for your plan, we will not charge you more than your insurance will pay.
Do you handle medical emergencies?
No. For any serious emergency, call 911 or go to the nearest emergency room. Once you are safe, please contact Dr. Polivick, and she will coordinate with the hospital team and monitor your care through recovery. The hospital team will be in charge of your care, but you won’t have to navigate it alone.
Can existing patients of Dr. Polivick join?
Yes. Dr. Polivick’s current patients are warmly welcome to continue their care at Foundation.
Due to a contract, Dr. Polivick cannot approach her current patients and ask them to continue care at Foundation unless they contact her first and express an interest. So please do not be disappointed if you do not hear from her directly. Dr. Polivick would love to continue to be your doctor.
Can I see Dr. Polivick before Foundation opens?
No. Until December 19, 2026, Dr. Polivick practices medicine only in her current position and will not see patients, offer consultations, or give medical advice in any other setting. Foundation will begin providing care when the practice opens in January 2027.
Enrolling as a founding member reserves your place in the practice. It does not include any medical care or advice before opening. If a health concern comes up in the meantime, please continue with your current physician, or call 911 in an emergency.
